Booking your birth with us

So that we can make your first appointment to meet us, please complete the self-referral form providing as much information as possible. Questions marked with an asterisk are mandatory.

Details of your midwife booking appointment will be emailed to you within seven working days. Please do not contact the booking coordinator unless you have not heard from us in seven working days.

Please note, any referrals submitted between the 21st December 2023 and 1st January 2024 will not get a response until after the 2nd January. If you have not heard from our team by the 5th January please call our booking coordinator. 

If you have difficulties completing this form, or have not received your midwife booking appointment, contact the booking coordinator on 01483 464004.

If you have any urgent pregnancy concerns e.g. bleeding, pain, please call 24/7 advice line Call a Midwife on 0300 123 5473.
