Discharge Information for Patients and Carers

We want to help patients on our wards get back home, or to their usual care setting, as soon as they are well enough. This is important for improving recovery and wellbeing.

If you are one of our patients, a dedicated team of healthcare professionals will work together with you to plan your discharge from hospital right from the start of your hospital stay. This team includes doctors, nurses, social workers and other specialists. Their aim is make sure that you are safe and well supported upon leaving the hospital.

Planning ahead for your discharge helps to keep your stay in hospital as short as possible and ensures your continued care and recovery once you are back home.
