Hospital at Home

The Hospital at Home team provides frailty care for people in their own homes, nursing homes or residential homes. 

  • Our team and a hospital consultant will look at your healthcare needs.
  • They will create your personalised plan and agree this with you.
  • They will advise what to expect during your treatment at home with us.
  • Whilst receiving treatment you will remain under the care of your hospital consultant.
  • You will be discharged from the service once your treatment is complete.

Service details – what happens when I’m a Hospital at Home patient?

A member of the team will aim to contact you within two hours and a member of our team will visit you at home.

You will be given a full medical assessment which may include some tests. If you require any additional tests these will be arranged for you.

Every day your case and medical care will be reviewed by our multi-disciplinary team.

Any changes in treatment and new investigations will be started by the team and take place in your home, where possible.

You will be updated each day by the team as to what is happening with your care, progress and treatments.

As a carer what can I expect?

The team recognises carers as equal partners in care, who provide vital information about the person with care and support needs.

The team will aim to include you in decision making regarding all aspects of the Hospital at Home care - from discharge, planning and support, to whether the person with care and support needs should remain in Hospital at Home ward if their situation changes. 

Your wishes and opinions will be respected in terms of care you are able and willing to provide, including the right not to provide care if you are unable or unwilling to do so. We will signpost you for available help, support and information including a carer’s assessment, if needed.

Who is eligible for the service?

  • Registered with GP in Guildford or Waverley.
  • Over 18 years.
  • Clinical Frailty - at risk of acute hospital admission within the next 24 hours.
  • Living in own home, residential or nursing home.

You will be referred into hospital at home by your hospital consultant, your GP, A paramedic and Other health professional.

Meet the team

The Hospital at Home team is made up of:

  • Registered nurses
  • Consultants and speciality doctors
  • A service co-ordinator (if the clinical team are not available)

Other team members include occupational therapists, physiotherapists, rehabilitation assistants and pharmacists who will contact you as needed.
