All pregnancies should be booked with a midwife by 10 weeks to ensure you receive all the health information and advice you need in those crucial early weeks of your baby’s development.

There are 5 options for how to book your pregnancy with us:

  1. If you live in the Guildford & Waverley area, please make an appointment through your GP surgery for the midwife booking clinic.
  2. If you are ‘out of area’ but live in Liphook/Liss, Pinehill, Petersfield, Bentley and Midhurst areas, please ring The Forest Surgery, Bordon and make a ‘Royal Surrey’ midwife booking appointment on 01420 477111
  3. If you live in any other area, please ring the Antenatal Clinic and make a booking appointment on 01483 464004
  4. If you would like to birth your baby at home with the Surrey Hills Home Birth Team, please call the team directly on 07467339395 or via email tanya.ashton@nhs.net
  5. If you would like to book for private obstetric care with Mr Osama Abughazza, please call his secretary Miss Ann Briant on 01483 442786 or by email ann.briant@bmihealthcare.co.uk to make a booking appointment.

You will need to complete the short form below BEFORE you make this appointment. Failure to do so, will mean the midwife is unable to undertake your booking assessment and your 1st ultrasound scan & screening tests may be delayed.

**You will need your NHS number to register for maternity care. If you do not know it, please contact your GP**

You will receive an appointment for your 1st ultrasound scan in the post AFTER your midwife booking appointment. If you have not received a letter within 14 days, please contact the Antenatal Clinic on 01483 464004.
