Congratulations to the Integrated Respiratory Service (IRS), who have been named finalists in the prestigious HSJ Awards.
The teams Winter Capacity and Demand Project has been shortlisted for the Clinical Pathway Redesign category.
The project resulted in more than 1,200 bed days being saved by preventing unnecessary admissions through proactive management of patients at home. This included a 58% increase in community home visits and assessments, as care was shifted from the hospital into the community.
As a result, the project contributed to a 16% reduction in COPD admissions and a 13% reduction in Asthma admissions.
