Care Home Support Matrons

Following the Enhanced Health in Care Homes National Framework, we are part of a team comprised of professionals working collaboratively to provide joined up care for Care Home Residents across Guildford and Waverley.

Who are we?

We are a team of two community matrons with advanced skills and vast experience in both primary and secondary care, working with a wider team including local hospices, pharmacists, local authority, the Clinical Commissioning Groups (CCG), GPs and Royal Surrey.

How are we improving outcomes for Care Home Residents?

Residents of care homes have complex healthcare needs reflecting multiple long-term conditions, significant disability and advanced frailty. Care provided to this complex cohort of people is often fragmented with day to day care services delivered to a variable standard, often with high staff turnover and limited support from the wider health system. Our aim is to support and advise the care homes when these gaps have been identified.

Care Homes can contact us directly with any issues or questions they have.

What are our aims?

  • A reduction in unnecessary hospital admissions
  • A reduction in A&E attendances
  • A reduction in falls and trauma injury to cohort of patients
  • Provision of best practice and education update sessions
  • Increase in End of Life Care and Preferred Place of Care by assisting and identifying an End of Life Care Plan
