Community Rehabilitation Team (CRT)

The Community Therapy Team (CTT) is a small team of physiotherapists and occupational therapists with a range of skills and experience.

We work with people with a variety of conditions in their own home in order to help them achieve their rehabilitation goals.

Service details

We will work with you to help you achieve your personal goals.

These may be in relation to your walking, exercise tolerance and ability, activities of daily living, personal care and return to work.

We will work with you to help you set realistic short and longer term goals with an emphasis on self-management of your condition.

What should I expect during the visit?

During the first visit an initial assessment will be completed.

During this assessment the therapist will discuss your home set up and environment.

They will talk to you about your ability to carry out your day to day activities and discuss with you what your therapy needs are.

They will help you to identify realistic and achievable goals for therapy and put a plan into place with you to support you to achieve these.

Who is eligible for the service?

  • Registered with a Guildford or Waverley GP
  • 18 years of age and over
  • Referrals are accepted from healthcare professionals only

How to access the service?

We take referrals from any health professional.

This can include your GP, district nurse, community matron or specialist nurse.

How can I tell you what I think of the service?

We very much value your feedback and our therapists will ask you to fill in The Friends and Family Survey. This helps us to review our services and make any necessary changes.
