

The Community Rehabilitation Team (CRT) is a small team of physiotherapists and occupational therapists with a range of skills and experience.
We work with people with a variety of conditions in their own home in order to help them achieve their rehabilitation goals.
We treat a wide range of conditions such as multiple sclerosis, parkinson's disease and stroke as well as musculoskeletal conditions where you may have had an accident, injury or surgery.
We can also help with those who require an exercise programme or confidence building following a fall.
We will work with you to help you achieve your personal goals.
These may be in relation to your walking, exercise tolerance and ability, activities of daily living, personal care and return to work.
We will work with you to help you set realistic short and longer term goals with an emphasis on self-management of your condition.
We are not a long term service, however the length of our intervention will vary depending on your personal goals.
This can be discussed with your therapist at your initial appointment.
We take referrals from any health professional.
This can include your GP, district nurse, community matron or specialist nurse.
When we receive a referral it is triaged by one of our clinical team.
It is then prioritised, allocated to a therapist and an appointment will be sent to you in the post.
During the first visit an initial assessment will be completed.
During this assessment the therapist will discuss your home set up and environment.
They will talk to you about your ability to carry out your day to day activities and discuss with you what your therapy needs are.
They will help you to identify realistic and achievable goals for therapy and put a plan into place with you to support you to achieve these.
We very much value your feedback and all our therapists will ask you to fill in The Friends and Family Survey.
This helps us to review our services and make any necessary changes.
