Community Nursing

The Community Nursing team visit people in their own home for nursing care, support and treatment when their health or wellbeing prevents them from attending a GP surgery or a clinic.  

Each team is led by a district nurse with a specialist qualification who manages a team of registered nurses, associate practitioners and health care assistants.

We work closely with GPs, social care, local hospitals, hospices, mental health services, social prescribing, specialist nurses and therapists to help individuals to manage their own conditions and to be as independent as possible, remaining at home where ever possible.

Who is the community nursing service available to?

The service is available for adults aged over 18 who are registered with a Guildford & Waverley GP and who are unable to attend a GP surgery or clinic due to their current health or wellbeing.

This include patients living in their own homes or residential homes but not nursing care homes.

How can I arrange an appointment?

This would be through referral from your GP or following a discharge from hospital.

You are able to enquire about the service by calling the Community Co-ordination Centre on 01483 362 020.

Where are appointments held?

The community nursing service will visit you at home.

What will happen at an appointment?

The community nursing service includes:

  • Administration of timed medication, such as insulin or intravenous antibiotics

  • Care and support with indwelling urinary catheters

  • Symptom management and support for end of life care

  • Continence assessments

  • Assessments for long term conditions

  • Care and treatment of complex wounds

  • Pressure area (bed sore) prevention and management

  • Leg ulcer diagnosis treatment and management

  • Phlebotomy

 

What should I have ready for my visit?

Please share your personal details, giving your next of kin contact details and details of medication that you are on or any hospital discharge letters.

Provide the visiting community nurse with hand washing facilities and paper towels.

When is the service available?

We work 365 days a year working from 8am – 8pm each day.

Outside of these hours please contact the Community Co-ordination Centre on 01483 362 020 to speak with a nurse to arrange a visit.
