Adult Community Health Services

Adult Community Health Services support and care for patients in the community, maintaining their health, independence and preventing unnecessary hospital admission.

Royal Surrey Foundation Trust works in partnership with Procare Community Services who provide community nursing services.

We work alongside GP practices, ambulance services, social care and other care organisations to provide an integrated approach to care services.

The services we provide are all coordinated through the Community Co-ordination Centre, a single referral point for health and social care professionals to refer local residents registered with a GP in Guildford and Waverley.
