The Integrated Cancer Care Team are a cancer rehabilitation team who support people living with the effects of cancer and its treatments. We are part of the wider community team working with patients, carers, GP’s, practice nurses, clinical nurse specialists and consultants, to improve quality of life for patients living with a cancer diagnosis.

The service provides personalised care and support through a variety specialist advice services, wellbeing events and interactive groups.

Who is the Integrated Cancer Community Team available to?

The service is available to adults aged over 18 who are registered with a Guildford and Waverley GP or who have been treated at St. Luke’s Cancer Centre at the Trust.

Who is the Integrated Cancer Community Team made up of?

Our team is made up of a cancer nurse specialist, an occupational therapist and a dietician.

What services are available?

The team offers:

  • 1-2-1 consultations with our cancer nurse specialist, occupational therapist and dietician
  • Interactive workshops covering topics such as: fatigue, anxiety, sleep, diet, physical activity, red flags, returning to work and more.
  • Every wednesday at 10am, an informal coffee morning to connect with other patients is hosted by a health care professional.

How do I access the services?

You can be referred by your GP or other Health Care Professional.

Or you can self-refer by contacting the Co-ordination Centre on 01483 362 020 and asking for the Integrated Cancer Community Team.
