Adult Community Health Services in Guildford and Waverley

Adult community health services focus on supporting and caring for patients in the community, maintaining their health and independence and preventing unnecessary hospital admission. They complement the services provided by GP practices, Royal Surrey County Hospital and other healthcare organisations.

Adult community health services include:

  • District nursing, community matrons and specialist nurses
  • Podiatry
  • Community rehabilitation teams including physiotherapists, occupational therapists and speech and language therapists
  • Rehabilitation beds in Milford and Haslemere Community Hospitals
  • Haslemere Minor Injuries Unit

Most of the care we provide will be in your own home, in a GP practice or clinic in the local area.

We are developing the way services are provided and we are focussing on keeping people well and treating them in the community and avoiding unnecessary stays in hospital.

About Procare Health

Procare Health is a Federation for GP practices in the Guildford and Waverley area. It was established in 2015 and includes 18 of the 21 practices but patients registered at any of the 21 practices will benefit from the new adult community health service. GP Federations allow GP practices to remain as independent units whilst providing an overarching organisation which enables practices to join together to take on joint contracts which benefit from the scale of multiple practices working together. These can include referral management services, delivering GP services in accident and emergency, extending GP opening hours, building better IT for general practice as well as supporting contracts like adult and community health services.

Royal Surrey is a NHS Foundation Trust based in Guildford. It is a district general hospital and specialist tertiary cancer centre. It serves a population of around 330,000 for district general hospital services and up to 2 million for cancer services.
