Chronic obstructive pulmonary disease (COPD)

Our multi-disciplinary respiratory team provides comprehensive inpatient and outpatient care for COPD patients.

Based within the hospital, we provide specialist respiratory outreach into the community, working collaboratively with GP surgeries and community services. Joint home visits are arranged when appropriate. Our strong links with community teams encourage a joined-up approach to care for COPD patients. This improves communication when patients come in and out of hospital.

Our services

Our services include diagnosis, education, exercise and self-management planning.

Our emphasis is on keeping patients well and at home by giving them information and support to cope with their symptoms and feel in control of their condition.

Management of severe or complex COPD may involve our team in supporting patients on home oxygen, home ventilation and providing links with palliative care when required.

Advice and admission

Specialist doctors and nurses are available to provide advice for patients and other healthcare professionals on all aspects of COPD or to take urgent referrals on week days.

Patients known to the team can self refer if they become unwell. Our team will arrange to see patients at home and are often able to prevent hospital admission. Patients who do require admission to hospital can often be discharged early with the support of our home team who will arrange home visits and care at home.
