Dementia documentation in practice

Within the Trust, work has been undertaken around behavioural charts and pain assessment tools with a view to develop a dementia care bundle for patients with dementia, delirium and confusion in the near future.

Work is also being undertaken around the development of a dementia care pathway and medical proforma for doctors to use when seeing patients. A delirium patient information leaflet has been developed and will be distributed in the near future.  90% of all patients over 75yrs successfully screened for dementia and delirium as per national recommendations.

 Patient centred care

The Royal Surrey County Hospital has worked with the Alzheimer’s society and specialists in learning disabilities to develop the This is me, My care passport. This document is pivotal in providing invaluable information about a person which will enable all staff to provide care that is compassionate and person centered.

The document can be used when caring for patients with a variety of conditions. A few examples are stroke, dementia, delirium and learning difficulties.  The, This is me, My care passport document can be found in all ward areas or can be accessed by clicking here.
