The Surrey Hospitals would like to improve their service to people with a learning disability.
They recognise that coming into hospital can be a difficult time, and would like to be able to know how they can help to provide you with service that meets your needs.
They now have nurses in the hospital who understand about learning disabilities .
To help you they need to know that you have a learning disability and you need extra support to understand :-
A) How the hospital will find out what is wrong with you
B) what they will need to do to make it better
C) What you can do to help keep yourself healthy and well
You may need the hospital to adjust things to help you when you come to visit. .For example :-
You can register that you have a learning disability and the help you will need by completing the form below and returning to the hospital you attend the most.
Your information will only be used to help you with your appointments.
