FOI 4022 Patient Falls

1. The number of patient falls that occurred in hospital in November 2016. 66 

2. The number of these falls that had a pre-falls multifactorial risk assessment. 66 

3. The factors that are assessed in a pre-falls multifactorial risk assessment

  • Previous falls in past 12 months
  • Any dizziness/postural hypotension
  • Any blackouts or loss of consciousness
  • Medications(if on more than 4)
  • Mobility & balance
  • Reduced confidence/fear of falls
  • Footwear
  • Delirium/dementia/Cognition problems
  • Continence(bladder or bowels)
  • Vision and hearing

4. Please could you categorise the number of falls by outcome in terms of harm severity?

  • No Harm – 55
  • Low Harm  – 9
  • Moderate Harm – 2
  • Severe Harm  – 0

5. Please could you categorise the number of falls by ethnic group? Royal Surrey County Hospital NHS Foundation Trust does not hold this information and are therefore unable to provide a response to this question.

6. Please could you categorise the number of falls by age?

  • Medium harm falls – Between 69 years & 80 years of age.
  • Low harm falls – Between 58 years and 87 years of age.
  • Royal Surrey County Hospital NHS Foundation Trust have not recorded No harm falls by age prior to February 2017

7. Please could you categorise the number of falls by time of day?

In November time of day were collected for the highest fall rate wards & were:

  • 07:00 to 12:59 – 12
  • 13:00 to 19:00 – 6
  • 19:01 to 00:00 – 5
  • 00:00 to 06:59 – 5

Low harm falls

  • 3 between 02:20 & 05:35
  • 3 between 09:00 & 10:10
  • 1 at 16:10
  • 1 at 19:00

Moderate harm falls were between 03:00 & 05:30
