As part of the work being undertaken by the Kent/Surrey/Sussex Patient Safety Collaborative Safe Discharge Measurement working group, we are keen to assess how incidents related to discharge are recorded on your Trust incident reporting system.

The purpose of this survey is to assess whether it is possible to aggregate incident data across the three counties in order to capture a meaningful picture of the issues being encountered within the discharge process and their impact on patients.

The Royal Surrey County Hospital NHS Foundation Trust (RSCH) uses the national Common Classification System (CCS). This is not unique to the RSCH site and is widely used by most Trusts as it adheres to the NRLS / NPSA data collection coding system. It cannot be amended, and as such, the details in the document below are identical to the options available to RSCH staff when submitting incidents.
