Intermediate Care (Falls and Home First)

Our community rapid response service provide short term rapid assessment and rehabilitation services to avoid unnecessary hospital admissions.

We look after patients with any types of infections e.g. chest infection, patients who have had falls and if they are not managing at home.

We provide assessment and support you with both health and personal care needs, including daily living tasks. We will help you with night sitters or night care if needed.

Areas covered:

We provide care and support in your own home. Based at Milford Hospital, our rapid response service covers the Guildford and Waverley community area.

How to access:

To access the service it is best to contact your GP or other health professional. They will write to us with all the information we need to ensure that you see the most appropriate member of our team for your particular condition and as soon as possible. While in some areas we will consider applications from patients directly, it does tend to cause delay as we often have to request further information from the registered GP.

Opening Hours

Our rehabilitation assistants will visit you at home and provide care and support depending on your assessed needs. Visits take place between 07.30am and 21:00pm.
