Adult Community Health Services in Guildford and Waverley
Adult community health services focus on supporting and caring for patients in the community, maintaining their health and independence and preventing unnecessary hospital admission. They complement the services provided by GP practices, Royal Surrey County Hospital and other healthcare organisations.
Adult community health services include:
- District nursing, community matrons and specialist nurses
- Community rehabilitation teams including physiotherapists, occupational therapists and speech and language therapists
- Rehabilitation beds in Milford and Haslemere Community Hospitals
- Haslemere Minor Injuries Unit
Most of the care we provide will be in your own home, in a GP practice or clinic in the local area.
We are developing the way services are provided and we are focussing on keeping people well and treating them in the community and avoiding unnecessary stays in hospital. You can find out more about how we are changing services here (link to Paula’s letter to stakeholders?)
